Partners Plus: An individual who purposes in his/her heart to make a monthly commitment of $50 or more to Mark Barclay Ministries.
Alumni Student: A returning student who is repeating the same course and fulfills all the academic requirements. (Note: No other discounts apply.)

I, the undersigned, do hereby agree to the following:

Based on my Christian integrity, I agree to honor the following requests of SMTI.

Not to reproduce any material in respect of copyright laws.

Not to lend or share any SMTI materials with any individual not registered as a student of SMTI.

I agree to the following tuition refund policy:

The down payment will be refunded up to one week after Orientation Day.

If my tuition is paid in full at the beginning of the school term and for some reason I am unable to continue to attend, a complete refund (minus the down payment) is available up to 30 days. SMTI headquarters must be notified no later than the fifth week of school in order for the refund to be issued (no exceptions).

I will be charged a $25 processing fee for any returned check.

I agree to pay all my tuition on or before the date due.

I understand that I will not be allowed to attend SMTI classes if I do not fulfill my financial obligations.

Tuition is due the first class day of the month. If payment is not received within 10 days, SMTI will charge a late fee of $25.

I hereby understand that all items related to the application submitted to Supernatural Ministries Training Institute are a part of the application process and become permanent property of Supernatural Ministries Training Institute and will not be returned.

I hereby state that all the information contained on my application is correct and true. If Supernatural Ministries Training Institute is notified that any information contained herein is false, it will be grounds for my immediate dismissal from Supernatural Ministries Training Institute. I also understand that completion of this application in no way guarantees or implies acceptance and/or enrollment as a student of Supernatural Ministries Training Institute.

I understand that the information contained on the Personal Recommendation and the Pastoral Recommendation is con- fidential. I hereby waive my right to see the confidential material contained therein, and I hereby release said materials to become the property of Supernatural Ministries Training Institute.
